Liability Waiver and Release: I, the undersigned, acknowledge and agree to the following terms and conditions while participating in activities at Vitality Fitness Studio: Voluntary Participation: I understand that my participation in any exercise, class, or activity at Vitality Fitness Studio is voluntary and at my own risk. Assumption of Risk: I am aware that participating in physical fitness activities involves inherent risks, including but not limited to, physical injury, medical conditions, or other health-related issues. I assume all risks associated with my participation in these activities. Medical Clearance: I confirm that I am in good health and have received medical clearance to participate in physical activities. I do not have any medical conditions that would prevent me from safely engaging in exercise. Release of Liability: I hereby release, waive, and discharge Vitality Fitness Studio, its owners, employees, trainers, and agents from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, injury, or death that may occur as a result of my participation in activities at the facility. Indemnification: I agree to indemnify and hold harmless Vitality Fitness Studio, its owners, employees, trainers, and agents from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney's fees, arising out of or related to my participation in activities at the facility. Rules and Regulations: I agree to follow all rules and regulations set by Vitality Fitness Studio. I understand that failure to adhere to these rules may result in the termination of my participation privileges without refund. Medical Treatment: In the event of an emergency, I authorize Vitality Fitness Studio to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for my immediate care and agree that I will be responsible for payment of any and all medical services rendered. Photography and Video Release: I consent to the use of my image in photographs and videos for promotional purposes by Vitality Fitness Studio. I understand that my image may be used in various media including social media, website, and promotional materials. By signing below, I acknowledge that I have read, understood, and agree to the terms and conditions stated in this Liability Waiver and Release Form.
